Miliatry Appearance: Spc Snuffy your daily appearance is substandard on a consistant basis. I expect you to look and act like a leader, however your appearance does not display that you are want that additonal responsibility of keep your uniforms and mannerisums to a professional level.

Job Performance: Spc Snuffy you are one of the hardest working soldiers in the platoon. You continue to accomplish any or all assigned missions without fail with litte or no supervision. Good Job. I would like you to start training other soldiers in the platoon on your knowledge about MOS related subjects and any vehicles that you’ve operated and are licensed on. This will start you on the training phase of being a good leader: trianing others on what you know.

Physcial Fittness: Spc Snuffy you recently took the APFT in which you failed all three events below the army standard. You scores were as follows: Push-ups: 40 Sit-ups: 52 and Two-mile run: 16:54. Spc Snuffy you are now required per platoon sergeant to conduct remedial PT to get you back to the level you need to be. I will conduct remedila PT with you to help motivate you to get your scores above the bare minimum. I strongly encourage you to conduct physcial training on your own time. Company level PT is menat as a sustainment phase, you the soldier are required to maintain a level of fitness suitable for the army.

Education: Spc Snuffy I recently enrolled you in the army correspondence course program to help you with points for promotion. Another program that great is the E Army U program. You can find out more about E-Army U by visiting your education center. Spc Snuffy you GT scores also could use improvent, I will get you enroll inthe Basic Skills Education Program (BSEP) to help you raise your scores.

Spc Snuffy I am not recommending you for promotion at this time because your not displaying the type of leadership that an NCO would display, It all starts with your appearance and how profesional you are. Spc Snuffy you are an excellent “hard working” soldier but you don’t display will and determination to provide a soldier with purpose, direction, and motivation.

Possible Plans of Action:

  • SM will imopprove personal appearance (look professional at all times)
  • SM will conduct PT ( remedial) with section chief or squad leader twice a week unitl next APFT. A Diganostic PT test will be given between the record APFT.
  • SM will be traing by NCO COC on how to dispaly leadersahip abilities.

  • Provide purpose, direction and motivation for the soldier.
